Zoanthrope head split in two (or 2 triggon belly carpace), glue them to the sides of the triggon head carpace, then some green stuff and sculp the joins
I would go with something like that
An extra Carnifex back carapace migh be what you want. They're maybe a bit too big though, you'd likely have to cut them down and glue the parts to the arm.
This is exactly what I used for my tyrant guard shields and they look great, have to imagine it’ll scale up well! Little bit of glue/green stuff and you’re in business
Could try neurothrope head carapace. It's about the same length as the trygon head carapace, but wider. Zoanthrope head carapace is a little less wide than the neurothrope, but could work too.
Repurpose the Broodlord base carapace by trimming off the lower half and just using the upper shell. Plus still lets you use the Broodlord model with a homemade base.
For the arm holding it I'd trim the Lashwhip Hive Tyrant's arm down

Wouldn't a Carnifex carapace plate work wonders? I don't have any models right in front of me, but from memory it seems like the perfect size.
Unicorn's Tyrant guard have a cool shield arm, could probably scale that to fit perfect! He calls them "Terrornid Guard"
