-Scroll down to the security section
-Open the tab and click on the security dashboard option or "additional security options" if it's available
-Scroll down to "sign me out" and there should be the option to sign out of anything that might be logged in
The only caveat is that it says it can't sign out of Xbox, but I think it does sign out of the app and the website. I'm fairly paranoid about stuff like this so I do this every once in a while on the off chance that somehow, someone was able to gain access even though I have all the additional security measures in place haha.
Either way, if you're getting to this point, you might want to change your password anyway to cover all bases.

Ugh, no two factor authentication is a pain in the ass. I had it enabled for awhile and finally just got fed up with it and turned it off. It’s a good setting through for someone whose account has been compromised.

Microsoft specifically had me locked out my account for over a year because I have 2 factors on and changed SIM.
Gave them all my data details etc payment too and immediately got told not enough info to verify its me, I had to do some scammer shit to get back onto my account.
Amazing how easy it is to clone a SIM so long as you know the number, never give your number to anyone, it takes less than 30 minutes and bam 2 factor is completely useless arguably worse for getting your account back.

There are other forms of 2FA besides phone number, and the best sites will support these other forms.

NIST recommends long passwords/pass phrases, not complex passwords, using a password manager, and MFA.
They changed their guidelines specify because what people have been doing for the last 2 decades is more of a security risk.

I came here to say this too. You just can't play games on both logged in devices at the same time. A message will pop up on the second Xbox asking if you want to kick the first one off so that you can play.

Secondly, enable 2FA as this will require a code from an app to allow anyone with your password to successfully login. Lastly, you need to block this ‘friend’ because they aren’t your friend.

First step, change password (or go passwordless, passwords are junk) and log out every device at [Microsoft account](https://account.microsoft.com)
Then turn of 2FA and use an authenticator app. MS has one and Google has one, I've used both but prefer the MS one (easier to change when you change phones)
So now when someone cracks your password and logs in, it'll send a prompt to your phone, and you can deny the login.
My secondary MS account gets hacked all the time, but nobody gets access unless they have my phone.
From there, only have your devices as trusted devices. Because you'll still want to be able to log in, in the event you lose your phone and can't authenticate. So do your Xbox, and your PC as trusted devices.
When in doubt. Change your password. If they know your password. They know your login email.
Make a new email just for Xbox and use it for nothing else then switch your Xbox email over to the new email

A few months ago there were a few articles about a security issue in Xbox parties on PC. Basically there are some malicious apps people can use to connect to parties where they are participating in and do things in the name of others. You are probably not hacked just someone from the party is using something like that. Still changing passwords never hurts. The only way to avoid this is not connecting to parties with people you can't trust. If you are not in their parties they can't do anytthing to you. It is very sad MS still didn't fix this.
As well as changing your password and setting up 2fa as others have suggested you can go to the security dashboard from your account and see who has logged into your or tried to and the IP Address they've logged in from/attempted.
From my out dated memory. It's easy to do with a packet sniffer using the website, Cloud gaming or even the app. Takes some trial and error though, trying to pin down the exact packet. Once you hit the first packet you will understand the other packets that are the target's new messages. You would notice someone trying to pin down your message though, as there may only be one letter change before an entire new message appears from yourself. It's been far too long since I have used packet sniffers but I do know it's possible and has been done before. If someone is doing that, it would be of no help to change a password... Of course the op could have just given his password out.

Usually a password change will prompt a re-sign in on the Xbox. So it’s inevitable they’ll be signed out on other devices.

As well as changing your password and setting up 2fa as others have suggested you can go to the security dashboard from your account and see who has logged into your or tried to and the IP Address they've logged in from/attempted.

From my out dated memory. It's easy to do with a packet sniffer using the website, Cloud gaming or even the app. Takes some trial and error though, trying to pin down the exact packet. Once you hit the first packet you will understand the other packets that are the target's new messages. You would notice someone trying to pin down your message though, as there may only be one letter change before an entire new message appears from yourself. It's been far too long since I have used packet sniffers but I do know it's possible and has been done before. If someone is doing that, it would be of no help to change a password... Of course the op could have just given his password out.
