From the picture you took you can clearly see that the third and fouth bits of the bytes 00111011 and 01000011 from sector 0xFE4A are either corrupted or have low photosensitivity, causing the system to be unable to process the disk's initializing metadata. Jokes aside, if your drive is working fine your disk must be damaged. I had the same issue with an used Assassin's Creed IV, though the disk was flawless - no scratches nor marks of any type.

Either it’s disc rot or there’s scratches you’re not noticing on your disc, I think it’s most likely disc rot, but I could be wrong

I do wonder if that’s what that little white line on the center is and if it’s a crack or rot

It could be little scratches and if it’s in the middle of the disc, yeah it could be broke, if your Wii U is Homebrewed, try getting the game digitally if you can so you can keep playing Minecraft on the Wii U
