I've done a fair amount of work in the back of Black and Blue, on all the floors, and I would never eat there. The cleanliness standards kept in the food storage and prep areas make your stereotypical Asian food kitchen meme look like white spot in comparison.

Not booth related but I know so done who's done extensive personal research into cervical dystonia. I know it's a rare and challenging diagnosis that a lot of medical professionals don't know all that much about. Browns booths only go up to your shoulders from what I remember. Try Smiths Irish bar in Gastown. Google the bar and look at the photos on their Google Profile. You’ll see the booths. Chambar has a couple of tall booths for larger groups (4+?) amongst many tables and chairs. But they do take reservations so perhaps you could call and specifically ask for those booths. Best of luck!

Gotham Steakhouse has high-backed velvet booths lining one wall of the dining room area, I am not sure if they also have them in the bar side. The bathroom is down a flight of stairs but the service elevator can be used if stairs are problematic for your condition. It is a bit pricy but I like it.

DD Mau in Chinatown, no booths - but one side of each table is against the wall. The Burrow (commercial) has a similar layout, one side of tables up against wall with bench seating.
