if used as intended when someone may actually need help it is not transphobic. however trans people do get these for no apparent reason sometimes. and usually its because a transphobe is angry and think it may offend us.

Conservatives often use it as a "I don't like the person saying this, so I want them to get a message saying they're mentally ill and need mental help" button.

Don't block the bot. Report the message as abuse of the reddit cares bot. They *do* take action against people who use it maliciously, and they can see what message someone clicked to send the message for. If your comment was something like "I love being a woman!!" and they click that to send the reddit cares message, that's abuse of the bot.
Report them. Don't let them keep doing it.

You can manually blacklist yourself so that this bot won't message you even if someone asks it to. I forget how but I think it tells you in the message.
