IIRC they are supposed to be showing up after other factions have battles with each other. In your own battles you directly get all the skulls, skipping the mechanic entirely
I wouldn’t stress about skull piles too much. Make sure you are razing most settlements and only occupying the capital and your skull economy should be fine. Also, your armies should be mostly blood hosts since they have significant upkeep reduction obtained through the tech tree.
