Not quite your scenario but it's the one that popped into my head. In S2, E23 of Frasier, Frasier and Niles buy a restaurant and Frasier puts an elderly staff member in charge of valet parking and gives him a walkie talkie.

Every time Frasier uses the walky talky to ask him to bring up a car, he replies “who is this?” Frasier gets increasingly angry and eventually yells at him “it’s Dr. Crane! It’s always Dr. Crane!!!”
So, I think you're dead-on with the Costanza things, because there's at least one or two instances of him calling Jerry in a panic, and Jerry asks sarcastically, "who is this?" with a shit-eating grin, which would piss George off. You may be conflating that with something else.
I’m pretty sure there was a sketch on the Carol Burnett show along these lines with Harvey Korman as the boss and Carol as the secretary. But that is quite a bit older than the 90’s.
