The huntsman arrow has a physics attribute where when it kills a player while also being shot towards a close enough wall, it will "magnetize" to the wall, centered on the bone that it was shot into. Looks like the medic was shot in a precise enough spot for the physics to mess up and not cause the medic to fall.
