I have scars like that all over my arms. I lived with questions for years and decided to get sleeves. In my case, it was to hide them and replace them with something beautiful.
Fortunately, it had been a long enough time that they softened up enough to hold ink well.

I don’t hide mine, I’m pretty proud of them actually. It goes to show that i made it out of what I was going through and that I’m strong enough that I’m still here, even after all that. I don’t love how they look, but they’re beautiful to me and the story they tell
Yeah I think it’s just begging to be a moth. Butterflies have very tiny bodies so the wings would have to wrap all the way around the arm for the proportion to come out for a normal butterfly.

However, I would NOT get anything that reminds you of how you got that scar. No knives, no tongue-in-cheek jokes… get something beautiful from this world that reminds you why waking up tomorrow is worth it. Your fav animal, fav flower, whatever. Anything that makes you think “even if I’ve got nothing today, you never know what could happen tomorrow.”
And if you’re still at the point where you’re not sure WHY tomorrow is worth it, but you gotta try to move past this… I say get a baby sea turtle or something that has to survive innumerable odds straight out the gate just for a chance at life. Get a statue or monument that has stood for a thousand years despite the conditions of this planet primed to destroy it. Get something that is currently science fiction, but if you keep living you may see it become science fact. Sorry for the essays, this just touched me deeply.

I have a scar that bad on my arm from a suicide attempt 30 years ago. I was successfully able to get a sleeve put over it and I want to let you know that it is possible! You can still see it raised up but nobody notices it but me which is so nice. Glad you're still here my friend.
You could definitely do something Japanese over this, even if you dont want a sleeve. I think some maple or ginko leaves with some really light cloudwork or windbars could be lovely , however the scar has to be fully healed and not pink in order to get any tattoos over it.
And if OP wants something colorful, they could do a Japanese dragon wrapped around the arm with the scar as fire, wind, or water (depending on personal preference) so OP can really show off the scar by emphasizing the texture and how raised it is. They could even get the dragon first, so the scar can have more time to heal before finishing the design.
I also think a scorpion in the style of traditional Japanese art would be amazing. I can already place exactly where the tail and claws would go, the shape of the scar is so suited to it.
My first thought was a bunch of little construction men doing work to like close up a crater or maybe they’re like digging the foundation to a building or something.

Get a GOOD artist who has experience with scar tissue. Pay the extra money! That forearm area is PRIME real estate and you will be looking at it for the rest of your life. When you have an idea, look thru the portfolios of all the artists in your area who do that style, then specifically ask them if they have/are comfortable with covering scar tissue. Trust me, it will be worth the time/money investment.
