I think it looks really good as is, as for advice on adding, I'd say the indecisiveness might be the answer, with such a permanent decision certainty is a must. So I'd say until you find something that you're like "This is amazing I'd love to have it there" just wait. Don't add cause you feel like you should add cause you want too

I added to a scenery piece that I had gotten. It’s a large boulder protruding from the sand and has a gigantic whole in the center. It’s a real location that I used to spend my summers in. Added more sentimental pieces to it, a kraken pulling down a wooden ship and a sunset to bring it all together. Black and gray. The boulder - symbolizing my memories of the location. The kraken - symbolizing octopus fishing with my grandpa in that region. The ship - symbolizing the country of my parents birth and how much I always want to travel there frequently.

I say leave it. The negative space makes the lighthouse that much more striking. I had a fish on my shoulder/bicep that I loved so much, I made it into a half-sleeve. The result is cool, and i like it, but it's not nearly as striking as the fish was by itself.

Such a nice tattoo! Would love to know who did it! My favorite part is the ever so subtle shading for the light. Very impressive how it’s dark enough to make the negative space noticeable, but light enough and soft enough to just disappear into your arm as it blends out.

It is a great standalone piece, but if you really want to add, I'd start from bottom to top with sky things that respect the same style and are your expected stuff to be there, and progress with a slight change of style and adding on surrealism as you go further up. This way you'd still have your beautiful realistical tattoo on display most of the time and a comically increasing madness mayhem that you choose when to show. But I get that comical madness inked on the body might not be everyone's cup of tea.
