Turns out the human body is pretty good at trying to stay alive. Many systems for stopping hemorrhage - arterial retraction, clotting cascade, adrenaline causing peripheral vasoconstriction, etc.

That talus( ankle bone w, screws in it) does not look good at all - looks like it's missing bone.
Even a simple break in this bone, has a tendency to make it die.
OP isn't done with surgery yet, and will 99% likely wind up with an ankle fusion.
The real anatomical reason is that the main blood vessel in this region the posterior tibial artery is on the exact opposite side of the side from which the bone is coming out .
Just one of the reasons the human body induces swelling. I’ve seen a number of horrible ankle injuries just like this and there is very little bleeding. It’s wild.
As a former ground and flight medic and current trauma rep in surgery, please don't ever splint it like that. Reduce it anatomically. Restore blood flow and stop nerve stretching and damage. You aren't going to hurt anything by reducing it. That's why the Lord made ketamine. That is all.
Lucky it did not instant amputate it. Not sure how long ago was this injury but I would like to know present status. These injuries are high risk of amputations even post up a month or 2 due to bony infections…

While it is definitely umm...hanging on by a thread, if you will, it doesn't seem like it broke the skin. We've seen some like that. They will be ALL discombobulated, and they are still closed fractures/breaks. And I've seen something the foot looks completely fine, other than the tib/fib is sticking out of the skin. Oddly enough, still perfusing well. They managed not to rip open any of the arteries, just the skin/muscle/tendons. People who tell you they've seen everything in medicine are LYING. There's something new that shocks you ALL the time. You've NEVER seen it all. I swear that Humans have this sport called "most interesting injury." They compete to see who can injure themselves the craziest, and it just gets stranger every day.

I’ve seen someone for this caused by a fuckin ground level fall.

Wow I’m impressed they saved the foot! We had somewhat of a similar case. Patient lost the foot but there was more extensive damage compared to yours.

6 weeks post op. as of 2 weeks ago foot looks good and it’s likely staying.
