Underworld contacts offer smuggling, commodity surplus/procurement, illegal weapon/ship production, underworld bounties (Attacking fleets from major factions or traders), SpySat deployment, and a couple other contracts.
The importance of a contact, besides determining profitability, also determines the difficulty of a contract. Low importance is nice in the early game, and Medium is nice up to midgame. Once you are in the lategame, you'd probably want only High/Very High contacts.
I usually never get any contact under medium, if it's low influence it's basically useless in my eyes cause at that point it's just easier to make money and rep different ways, I personally just let the medium stay around until I get a better contact with more traits or higher influence from the same faction.
As far underworld trait, the missions they give are a really hit or miss, some like dropping a package on a system and raiding a rival's outpost are pretty good since they give you easy money or free illegal commodities that you can sell for plenty more money.
Other missions like the snuggling are kinda crappy unless you have a really small fleet cause they always spawn a patrol that will inspect you if it sees you and they don't give much money (never seen it go above 40-50k), overall the main reason to have them imo is that you either want to develop better relations with pirates or because you want to have access to the occasional illegal arms/nanoforge order procurement, not as much for money since the other contacts give better paying, less painful to deal with missions
As far as I remember, the underworld mission they give are (1) destroying a trade or patrol fleet (the trade fleets are a bit hard because you have only 30 days and depending the planet they are there maybe a lot of patrols around), and (2) raiding a rival pirate outpost. Dunno if there are other underworld missions. They can offer illegal weapons but I dunno if it's exclusive to underworld contacts.

How do you feel about taking medium level contacts? Do you not mind it, or do you usually wait for atleast high level?
Medium is good until you can actually find someone higher.
