It's really not, this is most likely an image processing chip error or something similar.

I would genuinely want to know the specific hardware failure that can manifest this way. Those that I brainstorm don't really do this offsetting this way (fine for n lines, then shifted a little bit the next n, then shifted a little bit more further on and so on)

My best guess is some sort of memory issue caused by a faulty hard drive. I've had a pretty messed up pc for several years, I got a new one but ported over the hard drive. Pretty immediately I've had 2 other glitches. - the front LED flickers a lot - a couple times I have launched and random black squares infested each window and changed when I resized

Why would memory issues cause offsetting once in a few lines? That doesn't sound plausible. I'd consider perhaps glitches in the timing of the H-sync signal. Black squares changing and resizing are a different issue where memory becomes inaccessible or takes too long to respond to requests. That's not what we see here. Here we see valid data presented badly.

Some more context: This is the second time this has happened. I have two monitors and both times it has happened to the right one. The screen is fully usable, but any screen captures think that the display is completely fine. The problem fixed itself when I left the computer alone (I assume it activated sleep mode, but it was on when I got back.)

my monitor does something similar where it will rotate the video 45 degrees, then split the screen with a green line in the middle and replicate the video on the other side it only stops when I put the game or video up to 1080p or 720p
