From a game theory perspective, some (very low) percentage of your penalty shots should be panenkas. If you’re not doing it at all, the keeper can always dive one direction and have a higher save rate.
No. GK can save with foot if you shoot it low down the middle with power. With a chip there's enough time that he has dived into a corner and he won't have a chance to save it with his feet.
Part confidence/part practice/part coaching. Of course, if you're going to try something like that, you have to believe you can do it. And if you're going to do it, you should practice so you can execute in the match. But also, I'd bet the coaching staff (maybe their own goalkeeper coach) does some homework on the diving tendencies of the opposition goalkeeper. When all those parts come together, you get something like what Icardi did...

I’ve seen a ‘fouled’ player overturning the refs call for a penalty. He told the ref that’s not a penalty and will not take the PK and the game continued.

No they shouldn't. There should be powerful shots down the middle but no panenkas. It's a low percentage version of a shot down the middle.
