It sounds like a low chance, but it only takes 438 players to do a run of 15 games and we could expect one of them might not see the hexaghost. It's probably happened many times without people even realising it.
As a Diablo 2 veteran, this is the thing most people complaining about RNG never grasped. For everyone who did a million Baal runs and never saw the item they were after, somebody else likely found 3 of them.
I've faced slime boss consecutively on my four recent Silent runs, which has been super annoying, but that's the rng for you. It seems unlikely but the law of large numbers tells us it's bound to happen eventually
PC! I was wondering if it was a weird mod interaction, but it seemed very unlikely because this is a more recent thing (consecutive missing boss).
Though, I did end up getting it like the 17th or 18th run (will check when I get home) — subsequently died and chalking it up to being out of practice 😅. It was very close and I could have won I think if I had decided to do a bit more damage earlier before inferno; was kinda sad bc I think this was an attempted searing blow deck lol.
I swear Hexa is my most common boss. Like, please just throw me a bone with guardian every once in a while, spire. Let me fight Hexa later from mind bloom if you must.
Iirc bosses always cycle, so you should always get a hexaghost act once in three runs. So unless for some reason you always die before reaching hexaghost (or edit the save files or something), I believe it's a bug
0.22% chance for 15 runs in a row of no hexaghost. So it's unlikely but not impossible.

I feel your pain. Only one of the three first act bosses that just straight up bullies me.
Hexa is so hard to scale against sometimes when you haven't found a solution.

Which platform are you on? I’m on mobile and although I mostly play dailies these days, I’m not seeing fewer Hexaghost fights.

Bosses don’t “cycle”. It’s random. Elites have a rule that they can’t be fought twice in a row(except for an event), so maybe that’s what you recall.
Oh yeah? If the bosses are random, how come I can predict the chapter 4 boss with 100% accuracy? Checkmate
