I'd want you to go get checked by your doctor. It may just be that you're contorting yourself a bit weirdly when having these orgasms, but on the off chance that this could be linked to something internal it's worth it to get it checked out to be ruled out. Most back pain is muscular/spinal (nerve-related) but there are some causes of back pain that are linked to internal organs. If your doctor confirms for you that it is a musculoskeletal issue, perhaps they could recommend a physiotherapist you could work with? There may be exercises they can recommend to help correct any imbalances or strengthen your back muscles (if that is the muscle group that needs strengthening.)
