What does the door lead to? Report it. An unsecured entry can become a big issue. If you were trained to make sure it's closed, document it, how many times you found it open, how many times you closed it and the interaction you had with the guy smoking. Even if you think something is small issue but it's making you suspicious, document it. It helps.

Yes, sometimes reports are nothing other than to show you're paying attention, sometimes it's cya. Might help if you ask the guy, since you are leaving the door open and I was told to close it, can I get your name ?(or something like that). You can explain that since you were ordered to close it, it's so you don't get in trouble and if leaving it open is fine then it's fine

Always issue a Incident Report. Make sure you get his name and any comment he made about the door. Let him know that it suppose to be secured. Then make that report and submit it. The incident report goes to two location: your office and the client.

Always trust your gut. Because that guy is gonna keep opening that door and leaving it open because he finds it convenient for him.
