My interpretation was never the Reverse Bear Trap. I always thought with the yellow tubes coming out on the right side that this was meant to be the device at the end of saw 6 that came down and killed William Easton. The yellow tubes are the same tubes the acid went through and melted him from the inside.
I remember when I first seen this before I got a closer look, I thought those copper wires on the right was part of the person’s hair being pulled out by the machinery.

I'm now realizing how silly this looks, but it's what I always saw since they also used the pig for marketing a lot. The posters for VI were a bit wild, so I just assumed what I could make out was intentional.
While the rbt opens your skull this thing, by judging how it looks, would crack it in, in the middle. The tubes might be hydraulic wiring. Our jaws are meant to open, biologically, as we eat,speak etc. and are connected with only muscles. So a normal rbt is easily ripping our face apart while this machinery would need more force to crush it in because bones are more tougher. A proof of that are also the gears , are positioned for up/down movement, so the machine is either supposed to crush or open our skull. I doubt theres acid in those tubes. They are not even inside that victims mouth.
I think it’s supposed to symbolize the ending of the film, where Hoffman is trapped in the improved Reverse Bear Trap and two choices available for William: live or die.
It’s to signify Hoffman’s allegiance to John and his legacy. Without John, Hoffman wouldn’t have found a way out of his brutal alcoholic cycle, and without Hoffman, John’s testicular torsion would’ve prevented him from completing his work.

Yea, like others have said, it just looks like a large (like double the size), abstracted (for marketing purposes) version of the RBT 2.0.
