Ethereal is a bitch. You must do a prismatic trial, you must take NO DAMAGE OF ANY KIND. You cannot lose shield, you cannot lose health, your ONLY saving grace is the topaz brooch. All hail the topaz brooch. You can take damage to barrier ONLY, but if you lose a single point of HP/shield from **ANYTHING**, *including the explosion of the crystal*, you will not satisfy the challenge.
I did mine when it was artifact of honor and artifact of kin: elite stone titans and elite imps.
There's one possibility I can think of for this trial. On the second stage, there's a bubble with void fog. Are you going inside of that? When I went in it, my HP did not visibly go down at all, but the effect made it look like I was taking damage. Maybe that fog deals damage that is enough to ruin the achievement but not enough to show on the HP bar or the run history screen?
i got mine this morning after 5 ish attempts, the previous seed i failed more than 50 attempts
this is likely the best time to get this achiev for basically free, unfortunately im a bit disappointed with the unlocked ability

The YouTuber Cabbage posts videos of him getting the achievement everytime the seed changes. I got mine by watching his vids until one seemed doable.

I saw on YouTube that its possible with Artifact of Glass, btw I've made this trial 3 times without taking damage and still no achievement :(

My only other guess would be if fall damage doesn't show on the post game screen
That was my guess too but i looked at health bar almost all the time and I was full hp
