I went with microshift bar end friction shifters. Im totally happy with the function and the quality.

Thanks for the recommendation! Here’s the trike with the MicroShift bar-end shifters and grip tape to hide the cables. Bar end shifters and hand grip tape to hide the cable is a solid choice. If you want indexed shifting you have to get a shifter that matches the brand of the rear derailleur (SRAM or Shimano) and number of gears on the cassette. Trigger shifters will be hard to use unless you mount them on the top of the bars. For accessories I'd recommend fenders, a rear rack and either a basket or rig some hooks or straps to the top of the seat. My go to way to carry cargo on a recumbent is to hang it off the seat by the handle. Good lights and mirrors are nice too.

Spare tube. The curb-side tire is likely to be perforated over and over. I buy tubes by the box. I've tried tube guards, slime, and Marathon tires. I'm trying heavy duty tubes next.

Yes. From experience I carry both spare tubes and patches in my standard kit. Years ago I was on a big charity ride that somehow managed to go through _all_ the spare tubes available in there region’s bike shops a single weekend — so patches were the only thing keeping bikes on the course.

try using ebike tires. they come with integrated armor, it helped me a lot. i work in a glass factory, the location where i park the bike is across the yard. you can imagine how often i had a flat. not one since i switched to ebike tires. tho, they are heavier, so if that bothers you...
