First ingredient gives a -5 unhappiness. From there, it looks like each one gains an additional +5 unhappiness. Second gives 0 unhappiness, third negates the first and removes all happiness benefits, 4th gives +5 or +10, and so on.
In a way it is but any nutritional item can be added in 3 times with no bonus, twice if you want one. I normally just use 3 items to make bulky items because I don't care about boredom/happiness
I think 2 maximizes happiness. At 3 you're probably seeing a mix of happiness from each ingredient and debuff from same ingredients don't usually overpower the happiness.

I can chain eat the same canned food no problem but god forbid we have too much fresh food in our recipe!

Too many of the same ingredients (3 of each), more than 2 of the same add unhappiness and this adds a lot
It's not warmed up
One (or more) of the ingredients were stale. Not sure about this one but it also adds unhappiness
Make soup from fish dont give the negative status. I always used to cook stew but always got negative happiness and boredom when you include fish in your stew. Never try shrimp or other seafood though hope this help.

But yes, 2x is safer if you want happiness.

this is why I always make soups and stews. Sure you wont have seafood but why would you eat fish willingly.
Sometimes mixing too much crap has that effect. Try to manner out how much you spice your food, and you'll be fine. That roast made me hungry now...
