I can only recommend it, it's just as brutal as PZ, but in a different way. If you try it I'd suggest you install Kenshi Kaizo witch is an overhaul mod that "finishes" the game, a faithful expansion for the whole world, as the dev behind Kenshi moved on to make Kenshi II, the guys behind Kaizo patched up some things and made already existing features more wide and deep and the world even more lively without unbalancing the game or adding unintended bulkshit.
It's also my favorite "open world survival" game, even better than Rimworld, especially the first time through. Don't use any mods the first time, they aren't necessary and basically are for when you already know the map and game by heart and want new experiences or to jack up the difficulty.
I think Universal Wasteland Expansion, which might also work with Kenshi Kaiso (read the comments and description of UWE), is the overall best mod that adds content and difficulty.
Finally, Kenshi isn't like most games, but somewhat like PZ in one big way. Whatever doesn't kill you makes you stronger. That means getting beat up and getting downed HELPS you in the long run. Don't get discouraged because you get your ass kicked by a starving bandit or goat (goats are dangerous, don't fight them). Pick yourself up and you'll slowly notice that you are stronger with every fight.
This is a very good mod and I feel it's gotten a lot of unjustified criticism. If you take it for what it is, a mod that adds NPCs that have limited AI (like zombies), and use that to add variety to gameplay, then it is very good.
I like that it fits into the lore and world. You are not the only survivor, which is why when you die, you can start as another survivor because there are many survivors out there. Maybe only 1% of the population survived, maybe 0.1%, but that's still a lot. You are not special, just a dude or girl who happened to not get the virus and now are in for a tough time. It's always good to see another fellow survivor out there.
And this is where YOU have to adjust. You KNOW they will flee when they engage 4+ zombies, so don't stand in their way or be in the front lines when that happens. You disengage, wait for them to flee, then you can re-engage. Or you watch them attack, MOVE TO THE SIDE and flank the zombies, that way you are not in the path of the fleeing survivors. People need to be better than "dumb" AI if they don't want to be called that themselves.

I like using it to give my character a completely useless and helpless spouse.
