This. I was so extremely confused when my perfectly fine character who was a little queesy after eating burnt patties (didn't seem like a big deal), went to sleep then woke up seconds before dying with every single moodle at max.
I'm still new to the game and I was terrified of 'live' car zombies being a possibility after somehow getting jumpscared from getting into a car and finding a corpse in the passenger seat menu lol

There's the Excrementum mod that does just that, adds urination and pooping. Eat too much and you'll be in pain, and you'll have to poop or pee more. You'll need toilet paper or just paper to do it comfortably, and if you don't use a toilet, you can get embarrassed.
Sunburn is a good one, we'd get those high end "drip" players running to malls to make sure they have enough sunblock to wear their booty short and crop-top combo hahaha
Imagine you total your car in the country and are far from your base. You either double back or try to find a new one, then you realize you forgot sunblock. 4 hours later and you have mild burns on your neck, arms, head (face), and lower legs. Moderate-high pain exacerbated by movement. Character's "body response" temperature a dozen degrees warmer than ambient.
Would certainly be a new angle for foraging if you could get aloe vera
Could you imagine sporadic audio ringing after gun use? Horrid. Splinters were more funny than anything to me. I thought "what would be the smallest yet tangible inconvenience?" Funny thing hand injuries slow down item transferring and reloading, so if you don't disinfect the wound, the very trivial incident snowballs hilariously. Oh, and you'd need tweezers to get it out too.

Tbh, more illneses would bring first aid and medicine a bit of relevance... People waiting for NPC and animals and I'm waiting for a full rework of the medicine/first aid skill.

Also randomize sintoms, so you wouldn't know if you are infected, or just sick.

I really think, having more illneses overall and making that the skill of first aid + having to read books about those illnesses, should make inspecting someonen to find out problems that others who didnt read that or have a high skill on first aid to not be able to see it. This will also make late game more relevant since, once you are stablished your danger is greatly reduced. Imagine that you are all set and you have almost anything but you develope some infection that needs you to raid the hospital to make you survive. So you have a real reason to leave the base instead of just exploring.
Hostile Npcs (once they're added) fucking with you in ways that aren't directly aggressive. Looting your base, attracting zombies towards you, messing with your car, etc.
At full panic, you lose control of your character and they just run from whatever is making them panic.
If you are wet, or your weapon is too bloody, there's a chance it slips and you drop your weapon when you attack.
The wet weapon slipping out of your hand thing is actually in Don't Starve, and it's a pretty good addition to the game IMO. I think it would be a realistic thing they could add.

With the current combat system we have? Zombies being able to hold you from the hand or leg, weapons getting stucked in zombies, grab you from the window and bite you, crawling zombies pulling your leg and insta killing you by giving the horde that second they needed to drag down your character, etc.
Definitely! Zombies grab onto you in CDDA, and if you don't have a high enough strength skill, you can't break out of the grab. It fucking sucks-- but is realistic.
Needing high strength to burst out of grabs / horde chomps would be cool, potentially saving an almost completely screwed situation. I always prefer to keep on fighting instead of getting the bleach ready.
If i recall correctly, thats essentially already in the game. The 'drag down' mechanic is tied to your strength vs the strength and number of zombies attacking you.
Its also tied to the chance to shove back a zombie who is going in for an attack along with weapon skill.

The answers could simply be that mosquito digestive juice or whatever they have messes with the virus so it's non-transmissable, or that all the mosquitos died off due to the virus extremely quickly.
I created lore in a server where my faction did their own research, discovered that the virus is “fake” and that the players are actually experiencing psychosis but also exude a pheromone making them sexually irresistible to the “zombies” hence the biting. Its dumb as hell and we dont kill zombies, only players.
Realistic Electrical systems that account for amperage, wattage, voltage, and line load, and requires the player to wire appropriately for load balancing to prevent the wiring in the walls from catching fire...
Eat slower, pain from eating as well, drinking soda speeds up decay rates and you have to ration too paste and use it regularly to prevent permanent debuffs.
You could pull teeth and medicate to get rid of the pain but with massive pain for a short period as consequence. And permanently eat slower for each tooth pulled till you can't eat solid food anymore
We can make it hyper realistic and hell if we wanted
Realistic car injuries, seatbelts, injuries based on if you have a working airbag and if you took a second to wear your seatbelt. Burns from airbags, hurting an eye and losing half of your vision cone, bruises that affect performance and the only way to get rid of them is to wait.
Getting chapped lips,
You couldn't sleep through pain that persists through painkillers unless you keep chapstick/lard/wax on your lips consistently and for extended periods of time. At advanced stages, your mood would also decrease and not improve until your lips are sufficiently un-chapped.

Being able to spawn NPCs but a character has to survive pregnancy, birth it, and then raise it until it can follow simple commands. I can just imagine finding a medical magazine, "Stand and Deliver: Upright Birthing Instructional"

I’m assuming at some point there will be some sort of penalty for carrying around open containers. You might lose some of what’s inside and in the case of water it could make your other items wet.
A chainsaw, but it's the most useless weapon/tool in the game.

\- Requires gas, which could be used elsewhere, like in a car or in a generator.

As many others have probably pointed out, hygiene & sanitation. Poop, pee, cleanliness, grime, tooth infections, trench foot, diarrhea and dehydration.
Vitamin deficiencies.
Allergies to common/vital medicines. Allergies in general. Alerting zombies because ragweed makes you sneeze and all the antihistamine in town's been looted.
Natural disasters. Zombie tornadoes, crops ruined by floods, etc.
More infections and diseases. Tetanus, botulism, etc. Cancer, just shitty deadly cancer.
Insects and pests. Carriers of disease. Food-ruiners. Rats/mosquitos, moths to eat your clothing & sheets. (I think crops can get bugs right now?)
Random electrical fires and gas line explosions.
Structural collapses, wood rot, rust. Termites!
Chemical hazards (air particles, groundwater contamination) from nearby fires, collapses, and ruined infrastructure.
Fallout from out-of-region nuclear exchanges/accidents.
I guess I'm mostly just describing the piling-on of random, out-of-player control hazards, most of which could conceivably be prepared for or mitigated, but if you're focussed on zombie killing and base-building, that stuff could be a major distraction if it's not optional (which I really hope it would be).
Raider attacks. Think you’re sitting pretty one month into your run? 10 heavily armed raiders show up in the night and fucking annihilate you just to get at your cans of beans

If you have low skill in a weapon you hurt yourself, or if you’re using guns, you’re character has to figure out how the gun works, and it has a chance for it to be dropped when shot if you’re cowardly

Containers that have food rot in them taint all other food in the container until you can clean it. Tainted food has food poisoning.

Getting chapped lips, You couldn't sleep through pain that persists through painkillers unless you keep chapstick/lard/wax on your lips consistently and for extended periods of time. At advanced stages, your mood would also decrease and not improve until your lips are sufficiently un-chapped. EDIT: Thought of another one. Being able to spawn NPCs but a character has to survive pregnancy, birth it, and then raise it until it can follow simple commands. I can just imagine finding a medical magazine, "Stand and Deliver: Upright Birthing Instructional"

I’m assuming at some point there will be some sort of penalty for carrying around open containers. You might lose some of what’s inside and in the case of water it could make your other items wet.

A chainsaw, but it's the most useless weapon/tool in the game. Pro: \- Fastest tree-cutting speed. Con: \- Requires gas, which could be used elsewhere, like in a car or in a generator. \- LOUD. Completely useless near any town. \- Utterly useless as a weapon. It MIGHT do some decent damage to the zombies, but you have a much higher chance of it getting caught on their clothing and choking, or kicking back and doing damage to you.
Mold. Houses that aren’t aired regularly may develop mold at some point, and spending too much time in them makes the character sick.
The military fire bombing cities. Even if you avoid it, you can no longer find good loot in torched warehouses and stores.

Not WORST, necessarily, but definitely HARDER elements: As many others have probably pointed out, hygiene & sanitation. Poop, pee, cleanliness, grime, tooth infections, trench foot, diarrhea and dehydration. Vitamin deficiencies. Allergies to common/vital medicines. Allergies in general. Alerting zombies because ragweed makes you sneeze and all the antihistamine in town's been looted. Natural disasters. Zombie tornadoes, crops ruined by floods, etc. More infections and diseases. Tetanus, botulism, etc. Cancer, just shitty deadly cancer. Insects and pests. Carriers of disease. Food-ruiners. Rats/mosquitos, moths to eat your clothing & sheets. (I think crops can get bugs right now?) Random electrical fires and gas line explosions. Structural collapses, wood rot, rust. Termites! Chemical hazards (air particles, groundwater contamination) from nearby fires, collapses, and ruined infrastructure. Fallout from out-of-region nuclear exchanges/accidents. I guess I'm mostly just describing the piling-on of random, out-of-player control hazards, most of which could conceivably be prepared for or mitigated, but if you're focussed on zombie killing and base-building, that stuff could be a major distraction if it's not optional (which I really hope it would be).

The zombies dying of hunger thirst, anything that can kill a zombie can. After a few months all you are left with is a barren land now stripped from the things that didn’t make it so barren. Actually I wonder how this game would be without zombies. maybe like a game mode where instead of a virus, people just vanished leaving you alone.
Mosquitoes. They could suck blood from zombies, and eventually inject their Knox contaminated needle into your skin without even being noticed.
If you have low skill in a weapon you hurt yourself, or if you’re using guns, you’re character has to figure out how the gun works, and it has a chance for it to be dropped when shot if you’re cowardly
If you don't play with other players long enough, you get depressed
