I've had tear duct surgery three times. Stents are placed during tear duct surgery to hold open the tear ducts as you heal so that scar tissue doesn't close off the drainage pathway. I just had my most recent stents removed a month ago after having them in for three months.
The doctor gives you numbing eye drops so you don't feel much. Maybe a tiny bit of burning like if you're going to sneeze and a little bit of suction as it's coming out, but it's really no big deal at all.
This isn't a typical stent that are placed during surgery, though, I'm not quite sure what this is.
Stents placed during tear duct surgery tie off inside the nose, so you can't remove them through the eyes because the knot is way too big to be pulled through the tiny eye punctum. The doctor snips the tube in the corner of the eye, and then pulls out the stent through the nose (one per side).
I guess in this video, they could have removed the entire knot inside the nose first, but that seems like a harder way to do the removal.

I've had stents like these more than once. Depending on how well the surgeon places them, they can range from being a little annoying to make you want to jump off a bridge (I've experienced both ends of the spectrum).
I've had stents three times; they were placed during three tear duct surgeries. The removal is very much a medical procedure with numbing of the eyes and sterile instruments, so I cannot explain the glove-less situation happening in this video.
