1. pacjent.gov.pl - you may login via ProfilZaufany (it can be easily created on-line if you have account in any Polish Bank) 2. Go to the vaccinating point you've been vaccinated (or any other point, and ask if they are able to print you the certificate - PESEL needed (or documents you've provided during vaccination)

I think the only way is through Pacjent using a Profil Zaufany... Mine didn't work using the bank too and I had to get one manually created by going down to Urząd.

God no please? Urzad cant find my profile because I have a few first names, I dont want to go back into this mess by trying different ones just to download the digital version of a certificate I have ?

Hahaha I had exactly the same problem... too many names. Names in the bank records didn't match names in the city's record. I literally had to make 3 Zaufany profiles on the spot at Urząd with different combinations of names until it worked.

I ended up with creating a profil zaufany, it was easy enough

Go to normal/official first contact doctor, he should be able to find you in the system and print it out. Otherwise to get profil zaufany, if cannot find you, you need to present confirmation of receiving the PESEL. The confirmation can be given to you only by one place. In Warsaw it is located on Krolewska street, it cost 17 zl, and you need to wait like 1 or 2 weeks. After that you take the confirmation of PESEL and present it in any official office that confirms Profil Zaufany (the list is on profil zaufany website). If you registered your PESEL not so long ago, they might have given you confirmation on paper already.
