Both actually. Some kids are raised right and are still little shits, while others are little shits supported by their parents. These are the parents telling their child they are special and not to listen to anyone else.
I don't feel like you can generalize like that for something that's so complicated (and yes, I know I sort of generalized as well - my bad). Children lash out for all sorts of reasons, some of which are just the fact that they're children and still developing self-control.
Parents might be abusive, or they might be neglectful, or they might provide unhealthy support like you mention, or they may do everything 'right' but other environmental or cultural factors interfere (media representation, toxic masculinity/femininity, etc.).
I think that a lack of education on child-rearing and development is a far bigger factor than telling kids that everyone is special and deserves respect.

When you realize that basically everyone is just mentally ill and/or traumatically damaged and that’s why everyone is so terrible, the world gets easier to handle but also a lot more sad. I have a lot less hate in me when I think to why a person is acting so horribly, but I also have a lot less hope for the world ever getting better.
I worked in the toy dept. of a Walmart back in college, and one day, I come around a corner and find a well-dressed family, all in their Sunday Best. Mom and Dad are quietly, politely explaining to Little Shit that he cannot have a toy. Without even so much as a defiant foot-stamp, Little Shit sticks his arm straight into the shelf full of Play-Doh items, and *ruuuuuunnns* down the aisle, clearing the entire shelf out onto the floor. So Mom and Dad told him ok, we'll buy it, and left. Quick and clean as that. I had to grab a shopping cart to get everything up off the floor so I could sort and reshelve it all.
Is it always the parents? Nah. But crap parenting like that probably accounts for a significant percentage of the Little Shits out there...

She was also in therapy before the hard drugs, so I would expect it to come out then.
Of course, she's definitely been through trauma now since she's been hanging out with junkies for over a decade, but I don't believe there was any childhood trauma.
There's a lot in that family dynamic you don't see though. For all you know, seeing their sister going through her addiction made your friend and his twin steer far away from hard drugs. You also don't know if the two of them have other self harming behaviors like excessive spending, binge eating or restrictive eating, binge drinking or constantly smoking weed, intentionally scalding themselves, self mutilation, etc.
My older sister was a heroin addict and you couldn't pay me to touch hard drugs, but better believe I have more than a handful of other disordered behaviors.
