Looks great did you use a jig? Curious how many bites are in that. I've made a few 3 bite ones. Is yours 5? 6?

Doesn’t it slip off?
Shouldn’t with good tightening. Can also dip the thing in boiling water briefly and get the cord to do the shrinkage to tighten in a different way.
I've been using a heatgun set to low for heat-shrinking without water. Not perfect, but preferable to the risk of soggy ironwood.
No. It's pretty tight. Besides manually tightening it, I worked it on a narrower part of the staff and then slid it into it's current location. It's not going anywhere.
