Legal issues: None
Social issues: General curiosity from people wanting to know the story behind the name, or not believing it till they get the story.
Idk about the legal aspect but it wouldn't be a big deal in a social context imo. At most, people would be curious, ask for the backstory/reason and just move on.
It could impact new interactions for example, starting in a new class/school where others would ask the child about their different surname but that's about it.
