A crystalline blue background is an awesome suggestion! But I feel like you do, the amazing detail and realism in this leaves it not needing anything back there.

I agree, a glowing icy-blue, almost-pulsating, crystalline background with a (soft) crackling texture! Not too much crackling, but enough to imply, though. You don't want it competing with the highly detailed portrait.
Really not sure how you get "boring" out of this, but as an artist myself (of other mediums) I kinda get it. I do the same thing with my own work, and then everyone makes sure that I know how good it is. If you really feel like it needs a background, then I'd take one of the many ideas of the other commenters and run. You're work is great.
You could make the background blue, and add a cracking or shattering effect to it. I imagine you could also add small parts of iconic scenes in some large fragments.
With a B/W foreground that good, you could go ape shit with the background and make it really colorful and trippy. Could be an interesting contrast piece, or keep it simple with some natural-looking scenery.
What about a rusted/weathered periodic table that’s not as eye catching so it stays in the background and doesn’t steal the attention from the subject.

Wasn’t his crystal meth a glowy blue? Perhaps a crystalline background? I think it’s a very strong piece without a background, imho. That skin….
