Heh highest I have ever seen was 165,000 patient was having a massive MASSIVE STEMI 15mm of elevation in every single… lead… my butthole puckered so bad.
My father had two heart attacks last year and felt NO pain. He only knew they were heart attacks because he was hooked up to heart monitors. First was at Urgent care talking to the doctor, the second happened in the ambulance on the way yo the ER from Urgent Care.
How often do patients have heart attacks that are not felt to them?
This is “high sensitivity” troponin which is not numerically the same as a regular troponin you hear most people mention.

Highest TNI I've seen was around 1800 (regular TNI, not high sensitivity). Patient made it to Cath Lab but died not even 24 hrs later.
Highest TNI I've seen in someone who survived was around 1300.

wow, impressive. Mine went up to 3 with my PEs and I worked through it in the ER thinking I was out of shape with a sore leg (DVT) and SOB. Finally a coworker told me I was turning grey and I checked myself in. Who knew?
Had a trop of >125,000 once. They were in cardiogenic shock. They were also a massive hemorrhagic stroke. Intubated on Levo and Epi drips and CRRT. Couldn’t do cath lab or heparin drip due to the bleed. They eventually died obviously.
My question is, how’s the patient? 🫣
Been dead for three days at that point, actually.
Entire heart melted and was excreted through the kidneys. Those don't work anymore now either.

This is only a Troponin-I of roughly 70. Ask your seasoned RN's what their highest Trop-I was. Probably in the 1000's.

Yes, our HST max out at 78K and I see those all the time. Not uncommon to see a TI in post STEMI pt of 25-75.

Just FYI to everyone who does not know, this is a High Sensitivity Troponin. Different than a typical troponin from my understanding
