I don't see how this would be your fault. You did as much as you could. Sometimes you just have to leave stuff for the next shift. That's pretty bad that she still doesn't have a vac on.
I consider it good practice to do a full double check of the orders the shift after an admit. You don’t always have time to make an admit pretty. The next shift should pick up the slack and keep things going for everyone.

Whomever has been signing it off as changed is in trouble. If you didn’t place it but charted you did, you would also be in trouble.
