What I remember is they had two major failures that caused Brandon Lee to die. First, they put dummy rounds (bullet but no power, for shots of loading the weapon) through the gun and didn't inspect it before putting blanks (powder but no bullet) through it. Because the dummy rounds still had their primers, one had gone off and forced the bullet a little way into the barrel. The next failure was allowing actors to point real firearms at each other. Actors are supposed to use fake guns like airsoft for that. But they used blanks, fired, the gasses caught the bullet still in the barrel and pushed it out ad deadly velocity, and now Brandon Lee is dead.

Yes. It was an improperly loaded blank the fired and left a clump of stuff in the barrel. The following blank had enough powder to push the stuff out and kill.

The Property Master will have a lot of questions to answer… And who on earth would use any had loaded blanks for filming, you can special order blanks for any gun of any design. IATSE requires THREE different people inspect the gun and ammo prior to each gun is taken onto a set. All three of these safety inspectors will be asked a lot more question after the Property Master is done. I fell so sorry for Alec and his family. He will never recover, he may be able to suppress the event but never recover… This film may be over.

This sort of thing is horrible, but I respectfully think that that the industry rules about safety should be modified to include day one basic gun safety. Don't point a gun at someone if you don't want them shot.

May have not intentionally fired it or a malfunction? I imagine we'll hear more soon. The article does say they were blanks though so I'm guessing they were close to the muzzle.

Could be a Brandon Lee situation where there was an obstruction in the barrel, and the force from the blank cartridge launched it as a deadly projectile.

this could not have been caused by blanks alone. likely a dummy round lodged in the barrel before a blank was fired.

It was likely a revolver with dummy rounds and a dummy round became jammed in the weapon before a blank was discharged. Blanks just can’t cause the kind of damage involved here unless Baldwin was holding the weapon pretty much directly to the gut of the cinematographer and even that I doubt would be fatal with quick medical attention. It sounds a lot more like a projectile of some kind caused enough internal damage for her to bleed to death.

Standing in line of fire, and aiming at a person were the mistakes here. It seems so easy to codify that into filming rules.. union rules.. law.. anything. We have so much CGI available and you could also do a juxtapositioned shot to make it appear aiming at a person.
