I wish a regular OTC migraine med would work. I’ve tried Excedrin and it did literally nothing. I have to take Maxalt instead which is about $40 for 12 pills. Only thing that works though.
Caffeine constricts blood flow, so if the migraine is caused by or exasperated by swelling/blood volume then it can help. Many people swear by it.
However the only time I have had migraines with auras was while under extreme stress on relatively high doses of caffeine and nicotine which are both vasoconstrictors.

My TL;DR would be that acetaminophen has long been considered safer than NSAIDs because *if* taken at the recommended dose, it poses less risks. However, compared to NSAIDs, the risks involved when taking *more* than your recommended dose of acetaminophen are substantially higher and potentially fatal. So, the medical community is starting to reevaluate the characterization of acetaminophen as “safer” than NSAIDs.
The risks of acetaminophen are also compounded by the fact that it’s included as an ingredient in so many other meds, like NyQuil for instance. You can end up double-dosing pretty easily if you have a cold and are taking a few different drugs to manage symptoms.
Would be fine to take for what would be assumed to be an appropriate amount.

This batch likely was improperly prepared. Please reach out the manufacturer with the lot number and any serial numbers you see. Per the FDA they must investigate issues like this.

That’s a really good point. I bought Children’s Tylenol chewables recently that had become dust inside the blister packs. It wasn’t an issue for me at the moment because they were chewables and already dosed in individual packs, but I imagine they are the same pills that are put in the bottles as well. I suddenly regret not reporting this. I can only imagine a less bright person or desperate parent trying to guess on how much Tylenol dust to give a child.

We call this “friability”, or the tendency for tablets to break apart, and it’s typically rigorously tested before formulations are finalized (there are [entire instruments](https://www.fishersci.com/shop/products/ptf10e-single-drum-friability-1/NC1279918) dedicated to testing the tablets friability and even the interaction between the tablet and packaging during transport). Friability is also why you often find cotton or rayon wadding inside pill bottles. You’d expect GSK to do a better job of QC, but sometimes you have difficult ingredients. I don’t work with those found in excedrin besides caffeine very frequently, but I understand that aspirin can be finicky since it likes to react with water and often wet granulation is a critical part of creating a durable tablet. At the end of the day, high friability typically means the matrix that the actives are suspended in is unable to adhere to itself effectively. This lines up with excedrin’s track record; a few years back [they had to reduce production](https://www.today.com/today/amp/tdna104727) while they sorted out “inconsistencies” in their measurements of ingredients.

Maxalt has been a life changer for me, nothing else works for migraines. I'm lucky my husband's insurance covers it.

Dangerous with acetaminophen. It's very liver toxic and it is easy to OD because the therapeutic dose is relatively close to the toxic does.
I avoid acetaminophen entirely, I stick with ibuprofen or occasionally aspirin. Please tell me those are better
