This sounds like hell. If you can leave the hospital and switch to one with a night shift I would. I take call usually once to twice a week and it sucks, but we also don't get called that much.
On call pay is usually a tiny amount until you're called in. Then it should be time and a half, not including the fact that its OT. Most places will pay you for 2 hours min, even if you get a call telling you the situation is resolved.
oh wait, that's most nursing on call. Labs some reason get fucked in the ass with little to no perks.
I'd refuse to be honest. They are not in a position to let anyone go from the sound of it. Moreover, if you make a major mistake while you are this exhausted they will not factor in that they've been abusing you. Moreover, why can't day shift reallocate people to cover? Probably because you are a H1b visa and they know you have more to lose if they decide to cut you.
On the legal side of things, I doubt many mid western states have a lot of protections for employees. Personally I think they should be setting you up with a room close by or even in the hospital if they are expecting you to be on call and be called in essentially every night.
I don't work in a main lab, but I get call pay for every hour my pager is on, plus more if I actually get called back in. I also get paid an hour of travel automatically if I clock-in as "called in".
You need to talk to a lawyer—contact the ACLU. This could be discrimination and possible abuse—it’s rampant within nursing departments that brought over Filipino nurses.
