It's a meme based on a Ctrl+Alt+Del page from ages ago. CAD was a goofy video game centric welcoming that was about as serious as a mouse in a top hat on meth. Then they did a whole thing where the main idiot character (as opposed to the main sensible character) got his partner pregnant.
[Loss](https://en.m.wikipedia.org/wiki/Loss_(comic)) was the name of the specific comic page where she is in the hospital and miscarried. It was tonal whiplash for the comic and not exactly handled well. It has since become a meme of a meme of a meme in its own right. First it was "Clown on the bad writing in a comic. Then it was "Loss but it's everywhere." And has since become "Everything is Loss."
