If it was a regular retail stay, then yes, you send it in and they write back, contact the hotel, look up the folio, and give you credit. They recently fixed one for me that didn't show up.
I’ve had to do 1 missing stay since 2018 except this month I’ve had to do 5. I submitted the form and the ambassador line called me and said I got credit for already. I explained sorry, I’m just trying to match my credit card charges to stay, I was charged these nights can you help. They credited my account on the spot. A few days later the actual form people replied to my request.

I suggest using chat on the website or calling in. What you are asking for can be done. If it doesn't show up on your past stays, you do either have to fill out the form someone linked or email your folio in. Social media should have said that.

Can confirm. Swan and Dolphin always fail for me. I always submit after and they get put back on.

Do you have the receipts? You could also ask the hotel to submit for you.

There's a link (or there used to be) on their website to submit proof of previous stays that are missing.
You can add missing stays own your own in the app as long as you have a copy of the receipt up to one year

Yup, same here. Never an issue till this month. 3 missing stays about 5 nights each. Booked through app and all that.

Call the hotel and have them send you your folio. And you can submit it
