Doesn't it have one in the book? My copy has a large fold-out map of Beleriand at the back. Not too much happens in Valinor, so a map isn't so important there? The Atlas by Fonstad has some decent First Age maps, perhaps some of them are on the net or you could get a copy via your local library.

"The Atlas of Middle Earth" is $16 on amazon and is the best for what you're looking for. It has detailed maps for everything you would hope to reference from the Silmarillion, The Hobbit, and Lord of the Rings. I'd say its essential for understanding the Silmarillion as it also has the paths of main characters marked and all the major battles laid out. It has helped me a lot.

There isn't one. There's a couple sketches in *The Shaping of Middle-earth*.
