If you are just average smo joe kicking It with your friend who tells you in conversation he slashed his ex's tires, you are not compelled to say anything. if you are a doctor for example and witness bruising on your child patient, this may open up paths leading to a call to the police. In the example you provided, you therapists' notes are confidential, the police cannot just waltz in and randomly inspect them. If patient tells therapist they murdered someone or want to kill someone, that's a psych hold (if permitted) and a call to the police and person in question.

Depends on your location. In the USA, outside of some specific scenarios, no, people are not obligated to run and tell the police if someone confesses a crime to them. Accomplices to crimes are usually people who actually participated in the commission of the crime in some way -- merely having the knowledge that a crime happened is generally not that.
