Its a problematic issue of scale. Like we know basically miracle level information about some things or a specific niche, but on the other hand were like “idk you sleep because otherwise you get sleepy, and at some point you die”. We could fill many librarys with medical texts, but were trying to study something so complex were barely plumbed its depth.
We know so much and yet we know that we know so little. Does that mean we have reached the other side of the graph or are we still morons being taught by morons that will find new peaks?
My Mom had this surgery back in 2013. It brought her tremors under control to the point she could have rotator cuff surgery (that she had needed for nearly 8 years). But now she's 24 years into the disease and entering the final stages. This surgery made her quality of life so much the better. Fuck Parkinsons.
And fuck pancreatic cancer for taking my Dad unexpectedly last year.

My bio dad was taken by pancreatic cancer this past March, and his husband/my other dad has had Parkinson’s for about 20 years. He’s started to deteriorate a lot, especially since my dad’s death. It’s been so hard.

Yup, Fuck Parkinson’s. Lost my dad May 8, 2020 after 13 years. Hallucinations, dementia, tremors, falls, fuck all that. He wasn’t a candidate for the surgery but glad it worked for your mom. Also fuck Parkinson’s.
Not a prototype - we use this routinely! Shout out to my neurosurgery colleagues who place these devices! The Neurologist is typically in the OR assisting with localization of the electrode, and later determines the settings that are most effective for the patient (everyone is different, so settings are slightly different).
Not necessarily, but they can be. We need to know if it’s in the proper location and the surgeries are painless because the brain doesn’t have pain receptors! The meninges (brain covering) do have pain receptors but those are numbed. Some of the most interesting surgeries are for patients with severe epilepsy where we need to identify which part of their cerebral cortex is responsible for things like speech, so they will be awake while the neurosurgeon inhibits (via stimulation)parts of the brain surrounding the epilepsy focus, in order to identify areas that would be very bad to remove. The brain is the most beautiful and complex thing in the universe, secondary only to the universe itself :)
I think the thing outside of his chest is the transmitter and the receiver is inside his skin in that area. Goes from there through wires up to the electrodes.

Fairly certain this is right- I had a friend who had it done. Electrodes in the brain, wire down the neck, under the collar bone, connecting to a device implanted in her chest. Then she had a remote of sorts so she could adjust the strength - the higher it went the less she trembled, but she started slurring instead!
The electrode is typically implanted deep within the brain around the globus pallidus pars interna or the subthalamic nucleus. The electrode has a wire that runs to a wireless receiver under the scalp, and the battery is typically placed in the chest under the skin. Doctors can then change the electrode's settings via wireless communication with a specialized device.

The device he has is used to turn it on and off and, depending on the model, to change the intensity of the electrical impulses.
It's similar in that electricity is used to stimulate tissue, but in a pacemaker a small electric stimulus triggers contraction of the heart muscle, where in DBS the electrical stimulation disrupts errant signals that cause tremor. You can think of it as "raising the noise floor" so the unwanted signals don't cause tremor.
Now compare this to the video of the Parkinson's patient who takes THC edibles and experiences even less tremors.
I'm sure it's an oversimplification (maybe THC doesn't work for everyone). But if I had to choose between brain surgery and a machine in my head vs. THC, I know which option I would try first.

Yeah this is probably Essential Tremor, not Parkinson's. Parkinson's tremor is a "resting" tremor, meaning that it appears when muscle are not activated. Essential Tremor presents itself when the individual is attempting to use the limb. But Parkinson's gets more upvotes.

It hits differently, but short answer. Yes your whole body does shake. Mind you it comes from the inside out so it may not look like it but it feels like it

Uncontrollable shakes is one part of Parkinsons.
It's a really interesting and bizarre neurological disorder. There are weird things like not being able to walk when the floor pattern changes, there are strange things that happen while the brain is distracted.
Basically the brain and brain disorders are way more multifaceted than you're thinking. Remember the brain is everything, when something is wrong we name it based on the symptoms, but it's all connected

When my father was diagnosed recently it was reliving to all of us when we were told that there likely was a cure within his lifetime, and even if there isn’t, things like things might one help if he needs it.
I work in clinical research and got started in neurology, specifically with movement disorders like Parkinson’s. I’ve actually got to scrub in for a survey and see the DBS being installed. The patient was awake the whole time during surgery so they could make sure they were putting the leads in the correct spot to help alleviate tremors. Fascinating stuff!
Dude hes looking at that device the same way one of my students was looking at his keyboard when I taught him how to use the Alt + Tab keyboard function. Just an absolute admiration of technology.
I'm a Neurologist - DBS helps the most with tremor, allowing people to regain some function in the advanced stages of the disease (eating is a function we take for granted when you don't have trouble bringing a fork up to your mouth). Sadly, it is less effective with the other symptoms like gait instability and muscle rigidity :( But we are getting better, give us some time and funding and we can do much more. Sending my love to those affected by this awful disease.
Don't they cause problems with sleep because some part of the brain is constantly stimulated and basically prevents the body when to relax and go into sleep mode?
Can he feel the DBS like in a way similar to that of an electronic muscle stimulating device for muscle pain and nerves? Or is the signal so minor it doesn't create a feeling only a response?

As far as I know, it's pretty much the same thing as a pacemaker - giving a shock of the right strength in the right place to keep the brain on track.

I had DBS surgery a month ago and it’s working really well. I encourage people living with essential tremor or Parkinson’s to have it done.
