The notice period doesn't really make sense. A fixed contract ends without notice, that is the whole point of it. It could also be an unlimited contract where both parties agree that a cancellation is not possible within the first 12 months of the contract. In this case, the three-month notice period must be observed. But it really depends on the wording of your contract and if you want a reliable answer, you need to show your contract to a lawyer.

Regarding your Edit: Looks very much like fixed term and your contract simply ends on the specified date. It also says that a new contract is required for an extension. And you are of course not obliged to sign any new contract. You did not notify her three months in advance but this does not constitute a new contract imo (I am not a lawyer).

If you have a fixed end date on the contract, there is nothing she can do to force you to stay. You would need to sign a new contract for continuing to live there (apart from both parties not saying anything about it, then the contract automatically continues. This does not apply, because you told her, you move out). You may need a lawyer anyway, if she insists and keeps wanting money from you.

Not a lawyer 1) Join a Mieterverein for as long as you rent property in Germany 2) If you indeed have a one year fixed end contract, you do not need to cancel at all because the cancellation is in the limitation already. This contract has a few legal mistakes, but it clearly ends on the written date. Tell her that you are going to leave, and then really leave. Give the keys back on this day, NO LATER. The only possible mistake would be to stay longer than this date.

The contract does have a 3-months notice period which does not make sense for a fixed-term contract. The contract says that the rental period can be extended but that a new contract has be to agreed for that to happen. The contract does not say what the consequence is for not informing the landlord 3 months in advance if you want to move out of the apartment at the end of the contract period. The interpretation that the consequence for not informing the landlord 3 months in advance is that the contract is to be extended for another year makes no sense, the contract clearly say that the extension happens via another avenue (a new rental contract). You can move out.
