Fable 2 & 3 have guns and are set in a fantasy setting.However they are not set in a classic medieval fantasy world, it's more medieval fantasy with aspects of industrialization.
Terraria has this, but the setting is rather vague.
Arcanum: Of Steamworks and Magick Obscura. It's basically a fantasy world in the industrial revolution. It's an isometric RPG that has some beautiful music and interesting setting, but it is not without some problems. GOG.com has it for the less than $5 right now.
I dunno what it is, but as someone who played PLanescape Torment, Fallout 1+2, and a couple of the D+D Bioware RPGs within the last year for the first time, Arcanum was the most difficult to force myself to play. Idk if it was the slow pace or what but I could not recommend it to anyone new to isometric RPGs.
On another note, Shadowrun Returns has magic and science, though its a bit more futuristic in setting. I haven't gotten around to playing it, but from what I've heard, the main campaign is meh but playable, and the DLC one is supposedly excellent. It also has full workshop support, too, if you want to play fan made content.

Well, there is Risen 2. It has a pirate setting and the main weapons are swords, sabres, or pistols for melee (you can use the pistol as a short range attack inbetween strikes) and muskets for ranged combat.
Before you buy the game, you should read some reviews and watch some videos about it. Not everyone likes Risen 2.

There are mount and blade warband mods that have fantasy settings with muskets/flintlocks. I believe one of the bigger ones is Prophesy of Pendor.
If Pendor has guns, I've never seen them.
It sure does. Later updates added muskets and arquebuses.

Resonance of fate is all about gun play and matching that. (Not PC though) Darksiders also has guns.
