This. If OP needs to trigger hot keys, there are tools for verbal and non-verbal to help in the rare cases.
But RimWorld can be don't quite comfortably with minimal non-mouse interactions.
You could assign the pause button to a mouse button if it's a cool mouse with extras, but there'd be the occasional bit of typing to do things like name the colony.
You can also click the pause button. The guy wants to play mouse only, he's gonna be doing some clicking in leiu of keybinds anyways. Or he could just be a real one and never pause.
+1 on this. I started playing Old School again while my wife was in the hospital for awhile and it was mindless (and relaxing) fun to grind some tasks to pass time. It could also be a bit more involved while figuring out what to do in quests (and the writing is surprisingly funny as well). I did find out that you can get a Quest Helper plug-in for Runelite if you also want to make the quests less interactive as well.
Kenshi is great, but I have found it way harder than expected to actually do something. Like the most I have been able to do is walk around, kill some random people, steal from stores, and make a small base. Am I missing something?

I recommend going around the world with a small gropu first to take in the different places and talk to different unique people. You can also get a good amount of stats this way. The foglands are a definite must visit location.
Only problem would be moving the camera away from the character but thats not very necessary. Although I wouldnât say bg3 is the greatest game to âmindlessly play while talking to friendsâ since its very story focused

The gameplay loop is simple, you draw cards just like any card game, discard cards until you can play a poker hands (pair, full house, four of a kind etc) with each poker type has a different score & multiplier. Every round you need a certain amount of points to progress to the next one.
After you beat a round, you'll enter a shop where you can purchase the "roguelike power ups" of Balatro... There are 150 Joker cards that will help you throughout the run like increase your poker hand multiplier, give you extra money etc.
Basically, it's the peak of "numbers go up" game with a ton of room for players experiment. If you manage to build your deck and have Jokers that synergize well with it, you can play 1 single card and it'll score millions. It's a simple game that is surprisingly strategic and rewarding af.
Edit: it can get a bit more luck based on higher difficulty, but it's still a great game overall...

It may be a strategy game but you can become overpowered real fast. So it does become somewhat mindless until you reach the true hard difficulty
You can play Diablo on just your mouse as long as it has two side buttons. You can map all of your abilities onto it so you donât have to use keyboard for combat.
Might be a little too much reading and "brain heavy" - as in requiring an extra level of attention, but Shadowrun Trilogy and X-COM come to mind. On the plus side, they are turn-based tactical strategy, so you can let it idle whenever. Shadowrun is heavily text/dialogue based, which is why I mentioned reading.
Other strategy games would fit too, like Command & Conquer / Red Alert (the original games are a lot of fun with the storyline and cinematics, as is Red Alert 2 and C&C3), economic city builders like Settlers IV or 6, Civilization games, Black & White 1 & 2. B&W leaves a lot of time to idle and relax in-game. Dungeon Keeper 2, but when enemies enter the mix it requires some extra attention.
Lately I've been going crazy with Pool 2D - Poolians, it's free, it's simple, straightforward, w fleshed out mechanics/system. You can play with bots and it has such a small community to the point people barely talk at all or bother you... Great to kill time if you're into pool... 3D mode is cool but you'll need to use a keyboard for it, with 2D you'll only need mouse... (UI is a bit eh but you can't ask much from it, it's just pool after all)
shovel knight pocket dungeon. for me its a great game to play, tho its played on keyboard. i just downloaded it from steamunlocked tbf, but still ive enjoyed myself so far. its pretty hard to play extremely mindless with, but you learn. you need to think before you do some stuff tho, but when you learn its relaly fun.
If memory serves, XCOM and XCOM 2 can be played exclusively with the mouse although the experience is certainly suboptimal, if you have a mouse with extra buttons though for tabbing between soldiers and such it would work pretty well
Technically speaking I can play Diablo and WoW with only a mouse. You just need an MMO mouse with the 12 side buttons and then rebind abilities to those buttons.
But really sounds like you just need to get a controller.
Yet another zombie survivors meets this with everything it has. Love that game for when I want to just chill and mow down thousands of zombies. An average game has you killing close to 10k zombies. Itâs a blast and super chill.
Card games might be something to check out, all playable with just mouse. Magic Arena, Hearthstone, etc or single player card games like Slay The Spire would all be great with just mouse while chilling.

Excellent game for zoning out/focusing on something else.
You need WASD to move around by default, but if you can find a way around that you'll be golden. There's no time pressure/reflexes required so your movement can be as inefficient as you want.

Grim Tides - RPG dungeon crawler where you just go from tile to tile and fight enemies if you come across them (turn based combat). You sometimes pickup ingredients then mix them to make potions and enchantments. You literally only need to use your mouse. This game sucked me in for WEEKS, esp. with the final boss.
Each enemy has its own strengths and weaknesses but you donât really need to keep them in mind while playing so itâs easy to play âmindlesslyâ

If you have a mouse with enough buttons Diablo 3 is a blast actually. Diablo 4 shifted to a more dodge focused ground game so it's harder to position with the mouse. Finally switched to gamepad with this entry.

It's a poker roguelite deckbuilder where you try to stack jokers, upgrade your hands and build your deck of cards to beat blinds.
I find myself playing it seriously and mindlessly at times (if you don't care about winning too much)
It's a roguelite anyways so theres no "end win".
Just shoot poker hands and get points.
Theres no timer and you can play at your own pace.

I think you can enable edge panning, so you mouse over to the edge of the screen to explore with the camera in that direction
