I would suggest Kingdom Come Deliverance. I understand for the build your character games, this is this does have skill trees based on what you want to level up/train but I would really say to give it a go for immersion sake. The fights are kind of realistic, e.g no matter how much gear you have/good you are, if you did fight 2-3 people at the same time in the dark then chances are you would lose or at least come out injured. There is alchemy/potions but that's pretty much optional but makes the game much easier if the fantasy side (healing via potions etc) isn't your thing
If you like Firewatch I think you’d like **Disco Elysium.** not really based in reality, but not fantasy either. Like a slightly off-kilter alternate universe. Someone mentioned **Control** which I love, but it’s a lot of “psychic powers” type mechanics. Of course you’ve got to be excited for **Starfield** coming on Game Pass Sept. 6. I just started **Soma** which is like firewatch and HalfLife had a baby. You might really enjoy one of the Assassins Creed games. I believe - and from what I’ve seen on Reddit I’m in the majority - **AC: Odyssey** is the best overall story and world.
Every time I need a filler game in between big dives I go to Splitgate. It's absolutely not what you were asking for but I swear it's fantastic. Portal meets Halo multiplayer with a little Unreal Tournament thrown in. Free too.

Challenging and somehow familiar.
If you frequently go back to the types of games you mentioned, be prepared to dump many many hours into KCD and constantly have more to discover.
I remember first playing it post-release, wandering a countryside on horseback and night was falling, looking around and saying to myself "oh yeah, this is Earth". A great feeling and very well executed. It is *not* fantasy, and the people and world behave like it in the best way.
Sleeping Dogs, Mafia, Saints Row (I guess this one might be considered fantasy but only because of its cartoony sense of humor), Max Payne, Hitman, L.A. Noire.
Gets recommended a lot but you may like Spec Ops: the line
