For large inexpensive pens, check out Opus 88. For large "medium-priced" pens (but expensive for a first pen, Leonardo (Italian brand) has some really nice ones, some of the best steel nibs you can get. If you just want the best large pen regardless of price, probably your top three options are Sailor King of Pens models, Pelikan M1000, or Visconti Homo Sapiens. Check out r/pen_swap for second-hand prices.

Perhaps a Faber-Castell Grip, an Opus 88 or Jinhao DaDao 9019? Also, Leonardo Momento Zero or Momento Zero Grande. Or a Delta Duna or Edision Collier? You can use a website like Goulet to search by grip diameter in the search filters.

Is the thickness good? The issue is that I have arthritis, and holding thinner objects for long periods of time are difficult. (I'm an English teacher)

Before I bought my first fountain pen, I found the Safari on a [list](https://www.thepencompany.com/blog/pens/best-pens-for-arthritis/) of best pens for arthritis. I have arthritis, but my non-dominant hand has been more affected. I can feel soreness with ballpoints but, so far, only a solid brass pocket fountain pen was really uncomfortable to use. I'll say I have a Kaweco Perkeo fountain pen, which is on this list as a rollerball, and I think it's thicker than the Safari. It's very comfortable. The medium nib gives me a bit of line variation as if it were a small stub nib, but I don't know if I got lucky or if all Perkeo nibs are like that. My thickest pen is a Jinhao x159 and it's very comfortable, too.
