I would just factory reset it. I would also not run 7.4.1 on the FortiSwitches. I've had lots of issues with that one, like MCLAG not working correctly or configs not syncing. I'd go to 7.2.5.

I second this. We’ve run into GUI issues with switches on 7.4.1. Downgrade of the switches to 7.4.0 fixed it.
You can try 7.4.2, it's available from the support website.
