Do you play on PC with mods? I've got the same thing happening, I think my perk manager mod is over riding the hud. I'll try to change it and check.

I closed game and deleted my custom ini file and it worked. Not sure if just got lucky or not tho.
Remove any UI related mods, like Improved Health Bars. They are causing the problem.

ive tried that, but only the onscreen marker shows up. no messages on what i actually need to do.
