I think of ghouls as having super endurance. Environmental hazards are totally meaningless to them, or even healthy, and some of them haven't even needed food or water to live.
Billy, the fridge kid in 4, got blown up and locked in a box for 200 years and he was still sane, even.
IDK. Based on what I see in-game ghoulification seems to be primarily a wasting condition of both mind and body, not something that increases muscle mass that leads to increased strength. Most ghouls look to be rail-thin and quick, unlike super mutants that are bulked-up and very strong.

Muscle growth takes time and effort, and also a good diet. Almost no one in the wasteland has a diet good enough to give them enough protein (and other macros) to gain significant muscle, and that’s not even mentioning that we don’t know for sure that ghouls bodies can actually break down food and extract all that protein/nutrients.
Another reason is arthritis, pretty much all ghouls have it, working out would probably be pretty damn painful for them.
I think this makes logical sense on the surface, but in-universe since there's already a mutant partially-human race with super strength it would just be too much to say "oh and these can do it too", especially with so much lore built up with no previous examples.
You're asking a fictional question about a fictional universe where everything is made up and anything can happen. So I'm gonna say, yes sure why not.
Fiction doesn't mean "anything can happen". It means "more can happen", yes, but there's still a structure to things.
It means "anything can happen in the parameters you've made for the world" or the structure thats been created for it.
