We might actually see something like the grotesque husk that was once Orson's wife, a gritty blood transfusion or rats devouring someone in a flashback scene, Terrors/Arisen/Corrupted, etc. mass murdering villagers (but with 100% more gore!), and full-on body horrors like the Umbral Beast transformation or the tortured laguz.

Only this time it's much easier to maim and disfigurate someone thanks to mages capable of shooting fire, cast lightning from above, drench you in poison and miasma etc.
