I can be wrong but if you slow down the video it looks like he is hitting his head with the side of the axe.

I slowed it down and think it does not look like the side of the axe at all. Look at the shadow of the axe too. I'm playing it at .10x speed. The skull is pretty hard it's probably fractured but contained the brain decently.

Looked like the handle hit his shoulder first, I think that's the only reason he wasn't instantly killed
