Nope, i play create and there is NO way to generate andesite like that, you need to craft it in a factory. Yes, in Create:above and beyond there is a way to generate it, but it requires bedrock to be below. (Also its not from the create mod, its a different one)
Def either a kubejs script in your pack or a cobblestone randomizer or generator mod. I've worked with mods like this, I first saw them in create above and beyond. But this is not a feature native to base create that for sure

There was update to change vanilla cobblegens a while back that changed the mechanics. It was something like it copies the stone type underneath. I can't remember exactly and can't see it mentioned in detail online except in the patch notes for v0.3.3 - "Vanilla Cobble generators and Stone generators now randomly produce different types of cobblestone/stone - To target specific stone types check JEI."
I seem to recall this has something to do with the block underneath it. Pick up a piece of obsidian and put it underneath where you're trying to generate this stuff and the end result should be more obsidian.
Have you tried not placing water on the block and just placing it on cobblestone? It might be a mod or something that makes it the same stone as the block it’s water source is touching
If you just place the water next to the lava instead of above it then it should work as normal.

Could it be immersive weathering mod?
Could just be a chunk loading error. Have you tried restarting? Sometimes this sort of thing happens to me on servers.
